Does your primary care physician also care for patients
in the hospital? As options in healthcare become more complex and
time constraints more stringent, for many Americans, the answer to this
question is increasingly becoming no. This truth has created a need for
physicians that excel not only in delivering
outstanding
patient care, but also in communication and providing thorough,
detail-oriented
services.
Coupling these functions with providing cutting-edge teaching would help one understand the primary objectives of the H-Team.
Achieving all these goals have consistently
shown to improve patient outcomes. For example, emphasis on
communication with primary care referrers in the community has shown
reduction in important end-points such as medication errors.
The American Hospital Association lists these as some common types of
medication errors*:
- incomplete patient information (not knowing about patients'
allergies, other medicines they are taking, previous diagnoses, and
lab results, for example)
- miscommunication of drug orders...
- poor communication between inpatient and outpatient medical
providers
The H-Team is dedicated to including primary care
physicians
(PCPs) in the care of our patients. Our innovative
model allows us to involve and stay in direct communication with a PCP
in an effort to create a smooth transition from the hospital back to
their care as an outpatient. In addition to routine discharge
paperwork, we provide summative information to
your primary care physician in the event that immediate reevaluation by your PCP
is not necessary or available following discharge.
